Billing & Insurance

We participate with most local and many national insurance plans. However it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.

If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.

General Information:

• We accept most insurance plans. Please check with your carrier to be sure that they have our physicians listed as participating with your particular plan.
• Always bring your insurance cards to your appointment as we will make a copy for your medical record.
• Co-payments are required at the time of service. Please be prepared to make this payment before you leave your appointment. We accept cash, personal checks, debit cards and all major credit cards.

Self Pay:

• If you don't currently have insurance coverage, our policy is that payment is due in full at the time of service. We accept cash, personal checks, debit cards, and all major credit cards. If you are unable to make full payment at that time, we require a minimum down payment of $50 and further payment arrangements can be made.
• Please note that the $50 deposit is required and will be collected upon check in, at which time an uninsured waiver will require your signature and payment arrangements can be made. If you are not able to make this required payment on the day of your appointment, this appointment will be cancelled and rescheduled to a future date when you are able to make this payment.
• If you are in need of surgical services, a down payment of 50% of the surgical procedure fee is mandatory before the day of surgery.

Surgery:

• If you require surgical services at one of the local hospitals or outpatient facilities, please check to be sure your insurance participates with these local area hospitals and/or facilities. These include: Highland Hospital, Rochester General Hospital, Unity Hospital and Linden Oaks Surgery Center
